Use it to order Warrant Officer Memorabilia from the USAWOA Home Office.

  • Download the form to your computer before attempting to fill it out using the link below.

  • Do NOT fill out the form online as your personal data might remain visible on the web site for all to see. Use FILE/SAVE AS on your browser and select a folder to save the form.

  • Close your internet browser and open your word processor. You may use any word processor. Open the file "MemorabOrder_Fill-in.rtf." Press the INSERT key (normally top right on keyboard) to switch to "write-over" mode.

  • Square brackets ([  ]) indicate text entry points. Click between the appropriate square brackets and type and "X" to select an item or to type response such as for "Quantity," "Cost," "Name," "address," "Phone Number," etc.

  • When you have complete the form save it and then click on the "Send" link at the bottom of the form to bring up a pre-addressed email note to our secure email account. Press the INSERT key to return to normal typing. Close the word processor.

  • Attach the completed form to the email note. Place any desired comments in the body of the email. Send it. The Home Office will send you a reply acknowledging receipt of your form.

We recommend that you use your AKO or other military email for added security of your personal data. Use of commercial email to send the form as an attachment provides a relative amount of security for your personal data unlike text in the body of an email, or a text file attachment.

 

If you have difficulty with the above procedures, or do not want to use this method, download the form, fill it out using your word processor, and mail it to USAWOA, 462 Herndon Pkwy, Suite 207, Herndon, VA 20170-5235, or fax it to the Home Office at 703-742-7728.
